Charis Health Center is a faith-based, volunteer-staffed, primary healthcare clinic that serves only medically uninsured residents of Wilson County, Tennessee. Its mission is to serve compassionately the health needs of the working uninsured who live in Wilson County.  The grant requested will be used to fund a portion of the nurse practitioner's salary and benefits.  80% of diabetic patients with A1c's of 13 % + will decrease to 5.9-7% within 3-6 months.  80% of diabetic patients will have blood pressures below 130/80 at time of appointments.
