Founded in 2001, Faith Family Medical Clinic seeks to follow the commitment of people of faith to provide hope and medical care for the working uninsured by meeting their physical, emotional and spiritual needs. The vision of Faith Family Medical Clinic is that all working uninsured people and their families have access to the medical care they need. This grant will fund a portion of the medical director's and medical assistant's salary and taxes, as well as lab and pathology costs.  787 out of 1,050 patients (75%) who receive hypertension education to encourage them to take their medicines regularly, monitor their blood pressure and restrict their sodium intake maintain a blood pressure of less than 135/85.  480 out of 800 diabetic patients (60%) maintain their A1C level of less than 7% as a result of the knowledge that they should modify their diet, exercise and take medication. 150 out of 200 patients (75%) have a normal range of LDL under 130, Triglicerides under 200 and HDL above 40.
