Formed in 1978, Friends' mission is to ensure that all Hospital Authority facilities receive the grant and charitable support needed to leverage the precious tax revenue that provides patients with the health and safety resources they need to thrive. The navigator program will occur at Bordeaux Long Term Care, where the staff is devoted to creating a healing and loving campus for the community.  BLTC was founded in the late 1800s.  Its mission is to provide its residents with the highest quality intermediate and skilled healthcare, rehabilitation therapy and palliative care in a comfortable and loving environment.  This grant will fund a portion of a nurse navigator’s position at the Bordeaux Long Term Care Facility.  By the end of the grant year, 80 out of 100 (80%) of new patient admissions will receive appropriate counseling for aging issues resulting in an increased satisfaction rate from baseline of 50%.  By month six, 80 out of 100 (80%) of new patients will receive their medical assessment within two and one half hours of the time of their admission--speeding their ability to maintain wellness during their stay.  By month six, 40 out of 60 (67%) of patient families will send at least one member to attend their loved one's first care planning conference.
