At least 54 of 60 participants (90%) who receive health risk assessments will take at least one proactive measure to eliminate or reduce health risks within six months. At least 30 of 35 participants (85%) who attend weekly intervention classes will establish three to five wellness goals and will achieve positive progress in at least 50% of the goals by the end of the class series in order to improve their overall health.
