Mental Health Cooperative
Mental Health Cooperative, Inc., founded in 1993, assists persons with serious and persistent mental illness live successfully in their environments of choice with the least amount of professional intervention possible. They help individuals achieve success and satisfaction in their living, learning, working and social roles so that each can live productively outside the formal mental health system. This grant will fund a portion of the psychiatrist who is conducting mobile mental health outreach into the communities where homeless people live. The Mobile Outreach Program expects to provide 728 mobile interventions annually through the outreach workers, case managers, psychiatrist, registered nurse and/or the homeless services supervisor. 104 of 260 (40%) will follow-up with Mobile Outreach within 90 days of their initial encounter. 26 of 104 (25%) of those identified to have a mental health or substance abuse issue will be integrated into mental health and/or substance abuse treatment within 90 days of their initial encounter. 26 of 104 (25%) of those who follow up with Mobile Outreach and/or ongoing mental health or substance abuse treatment will move into some form of housing within 9 months of their initial encounter. 12 of 26 (46%) will maintain a stable living environment for at least 18 months after their initial encounter. 8 of 26 (31%) will experience at least a 10-point increase in their Global Assessment of Functioning (GAF) within 18 months of their initial encounter. Note: These will not necessarily be the same 26 individuals.
