Open Table Nashville is a non-profit, interfaith community, established in 2010, that disrupts cycles of poverty, journeys with the marginalized and provides education about issues of homelessness.  The grant requested in this application would be used to fund a bi-monthly overnight shelter in 5 locations which will serve as a means to build relationships with Nashvillians experiencing homelessness where service providers will meet them to dispense medical care, begin and follow up on the application processes for SSI/SSDI, and help provide identification documents, case management, and permanent housing.  Agency anticipates the following outcomes: At 8 months, 230 out of 400 ( 58 %) will show improved health (mental/physical/substance abuse issues) as demonstrated by the VAT and the VI.  At 12 months, 175 out of 230 ( 76 %) unhoused guests will continue to see improved health as shown by lower VAT/VI scores.
