The Primary Care and Hope Clinic's (PC&HC) mission is to provide quality, affordable family health services to medically uninsured, underserved, Medicare and TennCare families in middle Tennessee.  The clinic was developed in 1992 to increase the availability of health care services by combining the efforts of medical professionals and institutions, religious community, local resource agencies and community volunteers.  PC&HC's goals for the upcoming year include  1.) To provide access to preventative and primary health care services to uninsured, underinsured and/or underserved of Middle Tennessee through patient care delivered from three locations, one in the Gateway Development, one in the Murfreesboro Housing Authority and a third on a Mobile Health Unit through a collaborative between Middle Tennessee Medical Center and the Murfreesboro City School System. 2.) to provide a medical home for the uninsured, underinsured and/or underserved with an income below 200% of the federal poverty guideline , patients would see PC&HC as their medical home as indicated by unique patients to patient visit ratio indicating patients are returning for follow-up visits. 3.)Expand the health and diabetic education program to continue to allow face to face counseling sessions with a licensed dietitian for the uninsured, underinsured and underserved patients with diabetes, and other diagnosis which would benefit from dietary counseling.
