ProHealth provides quality, affordable and comprehensive health care services at each of its medical facilities for all members of the surrounding communities.   ProHealth opened its first not-for-profit medical clinic in Franklin, TN on March 6, 1996 to address Williamson County's need for expanded access to healthcare services for the uninsured and underinsured population.  This grant will support a portion of the office visit fee for the uninsured patients that access its clinic in Maury County. 147 (70%) of 210 patients identified with Type II Diabetes who are adherent with prescribed plan of care will achieve a Hemoglobin A1C less than 7% within 9 months after diagnosis.  1215 (70%) out of 1735 patients who identified as hypertension and who are adherent to prescribed plan of care will be at or below target blood pressures of 140/90mmHg within 120 days of initiation of pharmacological therapy. Those patients identified with a lipid disorder who are adherent to prescribed plan of care will achieve a total cholesterol less than 200mg/dL.
