Putnam County Rural Health Clinic
The Mission of this rural clinic is first to provide access to affordable health care for rural Putnam County, and adjacent rural counties, regardless of their ability to pay. Its second focus is health education -- encouraging healthy living by providing information and tools to assist in healthy decisions. The grant requested will fund the partial salaries for a nurse, a nurse practitioner, a quarterly diabetes clinic, management and follow-up of the Whole Person Health Service Database. 150 of 200 (75%) patients will report an improved quality of life as evidenced by a 2 point increase on a quality of life scale between initial program screening and 9 months later. 18 of 25 (72%) patients attending the quarterly Whole Person Diabetes Clinic will demonstrate improved diabetes management by a 1 point decline in their HgB A1c lab (an average measure of the patient's blood sugar level over a 3 month period) between initial screening and 12 months.
