Shade Tree Family Clinic
Shade Tree Clinic (STC), Vanderbilt University's medical-student-run free clinic, serves the underinsured and uninsured community of Northeast Nashville. Its mission is to provide high-quality comprehensive healthcare services while simultaneously training the next generation of physicians in compassionate and culturally sensitive community medicine. STC is managed and operated by volunteer medical students, and medical services are supervised by volunteer physicians from Vanderbilt and the greater Nashville community. The grant requested in the application will fund medications for patients with heart disease, diabetes, and chronic lung disease; social service evaluations; and health empowerment activities such as a diet and exercise group, smoking cessation group, and a disease education support group. 23 out of 30 (75%) participants in the STRIDE & TRIM programs will report an increase in knowledge of their chronic medical illness (STRIDE) or nutrition (TRIM) at the end of 12 weeks. 60 out of 68 (75%) diabetic patients will have reduced their A1c by at least 0.5% or be at goal A1c at the end of a 12-month period. 89 out of 148 (65%) patients with high blood pressure will show a reduction of 5 "points" from their systolic blood pressure (SBP).
