 While the fiscal agent for this grant in St. Thomas, this grant will be implemented by the Nashville Academy of Medicine.  Founded in 1821, the Nashville Academy of Medicine's mission is to be the leading advocate for its member physicians, its patients and our community, in promoting the highest standards of ethical medical practice, access to quality medical care, medical education, research, and community health. The grant requested in this application will be used to fund salary and benefits for the Bridges to Care Plus program.  Bridges to Care Plus will provide initial referrals for 350 new patients to volunteer specialists. This will result in approximately 1,960 patient encounters (based on an average of 5.6 encounters per referral.) The value of care donated from volunteer physicians will be approximately $801,640 (based on an average of $409 per encounter.)  Patients in the BTC Plus network will show a 45% reduction in unnecessary use of emergency departments, resulting in a significant cost-savings in the healthcare system. The Nashville Academy of Medicine and 2-1-1 will develop a formal Memorandum of Understanding between the parties to coordinate resources and develop a new system of entry into medical homes and Bridges to Care Plus. Bridges to Care Plus and 2-1-1 will cross-train all staff in assisting patients with enrollment options related to the Affordable Care Act, specifically working with Tennessee's Health Insurance Exchange.
