Alive Hospice offers loving care to people with life-threatening illnesses, support to their families and service to the community in a spirit of enriching lives. Established in 1975, Alive Hospice was one of the first hospice programs in America, and remains the only nonprofit, community-based hospice in Middle Tennessee. Coping with terminal illness presents many challenges, physical and emotional, to the patient and the family. Their caring, experienced professionals and volunteers work to meet those challenges, believing that no one, regardless of illness, age or ability to pay, should die alone or in pain. The grant requested in this application will be used to fund 70% of the cost of one LCSW team member who will provide bereavement support to patients’ loved ones and individuals in our community who are grieving the loss of a loved one. 594 of the 660 (90%) individual counseling clients will develop an action plan outlining how they want to better manage their grief based on their functioning. 535 of the 594 (90%) individual counseling clients will accomplish at least one goal outlined in their action plan resulting in an increased ability to manage their grief.
