Family and Children’s Service

Founded in 1943, Family & Children’s Service (FCS) serves all people in crisis and transition by meeting them where they are, understanding their needs, and providing them with the resources they need. The mission of Family & Children’s Service is to connect individuals and families to hope, to healing, and to one another. The grant requested in this application will be used to fund increased education, outreach, and enrollment services. This effort will target community-based entities with high touch assistance to: those newly eligible for insurance and an increasing number of people (who become known to us via outreach) who will be ineligible, have great need for support, and/or will become ineligible. 900 of 1000 or 90% of health insurance eligible clients will enroll in health insurance, improving their access to healthcare services. 900 of 1000 or 90% of surveyed healthcare navigation clients will be matched with a healthcare resource that meets immediate or ongoing healthcare needs. 930 of 1000 or 93% of surveyed healthcare navigation clients will report they have a healthcare home — a place where clients go for primary care and make linkage to the continuum of care.
